first thing to check would be a code reader for dtcs, failing that check the dpf sensor pipework for splits,cracks, burnt ends at dpf end. thats quite common. this can give dpf showing blocked when it isnt. the dpf sensor itself is another common fault that " fails within range" this fault doesnt normally cause a dtc so difficult to diagnose

Basically, what a successful regen needs is a sufficient temperature for a sufficient period of time. Temperature is created by feeding extra fuel into the DPF in the presence of free oxygen. The ECU just monitors the values which it sees. So you may have a fully OK regen, but the ECU may see a low temperature due to a malfunctioning temp sender.
